Cataracts aren't just a human problem — they're one of the most common causes of blindness in dogs, occur occasionally in cats, and even show up in horses. Dogs are where veterinary cataract surgery is by far the most established and successful, so this piece focuses mainly on them, with notes on how cats and horses differ. Unlike in human healthcare, there's no public system standing behind any of this — every route is essentially private, whether you're paying directly or through pet insurance — which changes the cost and "waiting time" picture quite a bit from the human version of this story.
What actually happens during the procedure
The technique is strikingly similar to the human operation: phacoemulsification. A veterinary ophthalmologist makes a tiny incision in the eye and uses ultrasound energy to break up the clouded lens before removing the fragments, then implants an artificial intraocular lens in the empty lens capsule and closes the incision with sutures. The big difference from human surgery is anaesthesia: because a dog can't be asked to hold still and stare at a fixed point, the procedure is done under general anaesthesia rather than local drops, which adds its own pre-surgical workup (bloodwork, sometimes chest X-rays) to check the animal is a safe anaesthetic candidate.
Before surgery is even scheduled, most patients need an electroretinogram (ERG) to confirm the retina still functions behind the cataract, plus an ocular ultrasound to rule out retinal detachment — since cataracts can hide underlying retinal disease that no amount of lens surgery would fix.
Recovery is more involved than the human equivalent, mainly because the patient can't be talked into following the rules. Dogs typically need a rigid plastic e-collar (soft cones and donut collars aren't considered protective enough) to stop them scratching or rubbing the eye, eye drops four or more times a day in the initial weeks, oral anti-inflammatory or pain medication, a harness instead of a collar on walks to avoid pressure on the neck and eye, and a series of rechecks that taper off over the following weeks and months. Done well, success rates are good: roughly 85–95% in dogs and around 90% in cats, provided the retina was healthy going in and inflammation is well controlled afterward.
What it costs in the US
Because this is private veterinary medicine throughout, cost is fairly transparent but adds up quickly. Cataract surgery for a dog typically runs $2,500 to $4,000 per eye, and since cataracts commonly affect both eyes, many owners end up budgeting $5,000 to $8,000 for bilateral surgery, including the pre-surgical ERG and ultrasound workup.
Pet insurance can offset a meaningful chunk of this if you have it — but only if the policy was in place before any sign of cloudiness appeared. Cataracts are one of the classic examples insurers treat as a pre-existing condition once symptoms are visible, so a policy bought after you've noticed cloudy eyes typically won't cover the surgery. This makes cataract coverage one of the stronger arguments for insuring a dog early, particularly for breeds known to be predisposed (more on that below).

There's no NHS equivalent in veterinary care — every pet owner in the UK is a private payer, whether directly or via insurance, for every procedure, cataracts included. As in the US, comprehensive pet insurance policies generally cover the surgery itself, the anaesthesia, medication, follow-up visits, and the referral fee to see a specialist — but again, only if the condition wasn't already present or symptomatic when the policy started. Because coverage limits and exclusions vary a lot between insurers, it's worth checking the policy wording (or asking the insurer directly) before assuming a diagnosis will be covered in full.
The "waiting list" question
This is where the comparison to human healthcare breaks down the most. There's no equivalent of an NHS waiting list, because there's no public queue to join — access is governed by how many board-certified veterinary ophthalmologists practise in your area rather than by rationed capacity. In the US, you don't strictly need a referral at all; many specialists will see a pet directly, though most owners come via their regular vet. In the UK, your primary vet typically does refer you to a specialist ophthalmology practice, but that referral isn't sitting in a public system — it's a booking with a private specialist clinic.
In practice this usually means an appointment within days to a few weeks rather than months, though the exact wait depends heavily on how many specialist ophthalmologists operate near you (they're a fairly small, specialised group in both countries) and how urgent your case is. A dog with rapidly progressing diabetic cataracts will typically be prioritised and seen quickly, since vets know how fast that particular type can advance (more below); a slow, stable, age-related cataract is far less time-sensitive. Insurance pre-authorisation can add its own delay of a few days to a couple of weeks if your provider requires approval before the specialist will book surgery.
The risks — and why age and underlying conditions matter
Diabetes is the single biggest accelerant in dogs. Diabetic dogs develop cataracts at a strikingly fast rate — roughly 50% within five months of a diabetes diagnosis, 75% within a year, and 80% within 16 months — because excess glucose in the eye's fluid gets converted into a substance that pulls water into the lens and clouds it from the outside in. Crucially, this happens even in dogs whose diabetes is well controlled; it isn't a sign of poor management, just a near-inevitable consequence of the disease. Because it moves so fast, quick referral to an ophthalmologist matters far more here than for other types of cataract, since delaying reduces the window in which surgery can still restore useful vision.
Breed and heredity matter more in dogs than age does. Several breeds are well known to be predisposed to inherited cataracts, including American cocker spaniels, Labrador retrievers, poodles, Boston terriers, Welsh springer spaniels, and Cavalier King Charles spaniels. Hereditary cataracts can appear relatively young — sometimes in dogs still well short of "senior" — while purely age-related cataracts tend to show up later and progress more slowly. Trauma and eye injury are the other common cause, unrelated to age or breed.
Surgical risks are broadly similar in kind to the human procedure — post-operative inflammation (uveitis), infection, retinal detachment, and glaucoma from pressure changes inside the eye — but management differs by species. Cats are notably more prone to persistent post-operative inflammation than dogs and need closer follow-up as a result, and in cats, cataracts are far less often caused by diabetes (which is rare in feline cataracts) and much more often the result of uveitis triggered by viral infections such as FIV, FeLV, or feline herpesvirus, trauma, or tumours — meaning the underlying inflammation itself sometimes has to be brought under control before surgery is even an option, and in some cats it never becomes a safe candidate for surgery at all.
Horses are a case apart. Cataracts in adult horses are most often caused by equine recurrent uveitis rather than ageing, and surgery in adults is uncommon — the eye is large relative to surgical equipment, the underlying uveitis complicates healing, and the intensive medication schedule (drops every four to six hours for weeks) is difficult for many horses to tolerate. Surgery is mainly reserved for foals with congenital cataracts or severe cases threatening quality of life, and even then outcomes fade over time: roughly 87% retain usable vision at one month post-surgery, but only about 26% still have measurable vision two years out, with at least three months of intensive aftercare and stall rest required regardless.
None of this means older or diabetic pets shouldn't be treated — cataract surgery remains the only way to actually restore vision once a cataract has formed, and success rates stay high in dogs and cats when the retina is healthy and inflammation is controlled. It's simply why a veterinary ophthalmologist will want a full workup — bloodwork, an ERG, an ultrasound, and in cats and horses, a hard look at any underlying inflammatory disease — before committing to surgery, rather than treating every cloudy eye the same way.
The bottom line
In the US, expect $2,500–$4,000 per eye without insurance, with insurance only helping if it was bought before any symptoms appeared. In the UK, the figure is £2,000–£3,500 per eye, with the same pre-existing-condition catch on insurance — and in both countries, there's no free public option to fall back on the way there is with the NHS for humans.
